The 24-week Healthy Lifestyle PlanTM is not about numbers on a scale, it’s all about helping you embrace a healthier lifestyle. We will focus on supporting self-sufficiency by providing you with recommended meal replacement through Bariatrix™*, a Healthy Lifestyle Cooking Series with Chef Jeremy, and the ongoing support you need to continue your newfound healthy lifestyle.

At Fairview, we know that there is nothing more important than your health and wellness. With over 60 years of medical weight loss experience, we’ve combined our knowledge and expertise to create the 24-week Healthy Lifestyle Plan. You’ll be surrounded by an academic-based, multidisciplinary team of dedicated experts to help guide you through your weight loss journey and equip you with the skills needed for long-term success.

Welcome to a Healthier Lifestyle

Before you begin, we’ll get to know you so we can create an individualized journey and give you the tools you need to succeed. This stage includes: 
  • An introductory meeting with your medical weight management team, including your provider and registered dietician
  • A welcome kit with a journal to help keep you on track
  • A thorough medical assessment by your provider and review of your medications, activities, and lifestyle
  • Determining your weight loss goals and developing a plan to get you there
  • Creating a personalized diet using recommended Bariatrix* meal replacement products to help deliver quick results
  • Discussing any other tools that can further help you achieve results

Start Anew

During this period, we’ll meet weekly to help you identify any barriers which may be holding you back and work to tackle each one through your personalized plan. This stage includes: 
  • Nine weekly visits with your Healthy Lifestyle coach to discuss your progress and concerns 
  • Two meetings with your registered dietician to ensure you’re seeing results and answer any of your dietary questions
  • An emphasis on learning to eat appropriately through structured meal plans using Bariatrix* meal products

Embracing Change

As you gain comfort with your newfound lifestyle, we’ll begin to transition your health into your own hands. This phase focuses on providing you with a supportive environment to ensure continued success, including: 
  • A provider visit to examine your current progress and assess any additional medical needs
  • Three hands-on Healthy Lifestyle Cooking Series classes at our Healthy Eating Kitchen
  • An introductory exercise and movement class to help you incorporate activity into your lifestyle
  • Three biweekly appointments with your Healthy Lifestyle coach

Learning to Thrive

You will have fully grasped the healthy lifestyle at this point and will be preparing to graduate. This last stage will focus on how best to sustain the results you’ve achieved. This stage includes: • Biweekly appointments with your Healthy Lifestyle coach. 
  • Two appointments with your registered dietician
  • One visit with your medical weight management provider where we’ll look back at your progress and a final health assessment will be completed at week 24, preparing you for graduation. This plan will be customized for your unique goals and will help you sustain your weight loss. 

EXCLUSIVE Healthy Lifestyle Cooking Series 

  • This course is only available to members of the Fairview 24-week Healthy Lifestyle Plan. 
  • You’ll attend three classes that are two hours each. 
Cooking classes are held at HealthEast® Clinic – Woodwinds, 1825 Woodwinds Drive, Woodbury, MN 55125

Throughout this fully interactive and hands-on series, we will build a platform of kitchen skills, from classic cooking techniques to knife skills to effective meal planning principals. Each of the three sessions will be focused on one of the three main components of a meal. This is a non-judgmental place where everyone is at home no matter what your skill level. 

Session One: Focus will be on both plant and animal protein cookery, from best cooking technique to safe minimal cooking temperatures. 
Session Two: It’s all about the incredible power and flavor of vegetables, as well as how to properly cook them. 
Session Three: Smart carbs will be the topic. Good carbs, bad carbs, proper portion, and cookery.
