Online: Go to MyFairviewRx.org, select 'Refill by typing Rx number' and choose 'Mail order' (not 'In-store') for your pick up preference. Or, register for an account for additional features such as automated refill reminders.
NOTE: Each family member requires a separate email address to log in.
Copayments may be made by credit card. Credit card information must accompany your prescription request.
Phone: To begin having your prescriptions delivered to your home or other location, call us at 1-866-377-6245 (toll free).
By Mail: Print out the Application Form - complete the information required and mail it with your doctor's prescription enclosed.
If you need to start your medication right away, please have your physician complete two prescriptions. One prescription needs to be written for a one-month supply to bring to your local pharmacy. The 2nd prescription must be specifically written for a 3 month supply by your physician. Mail the prescription for the 3 month supply along with the application form.
Call us at 1-866-377-6245. We will gather the required information and complete the transfer on your behalf.
Your medication will be sent to you in a plain package with a return address only. The pharmacy name will not be listed on the package.
You can expect to receive your medications 5 to 7 days from the time we receive your valid prescription order. If you need your medication sooner, call 1-866-377-6245 to discuss your options and make new arrangements if necessary.
When you order directly from our Mail Order pharmacy, there are no shipping charges.
