While living at one of our many partner long-term care or assisted living locations, you will receive the services of an advanced practice provider and collaborating physician through M Health Fairview Geriatric Services. This dedicated team of health care providers will see you in your senior living community and will coordinate your care. Our providers partner with patients, families and facility staff to provide a quality care plan. Our goal is to keep you as healthy as possible.
The advanced practice provider and physician through MHealth Fairview will serve as the patient’s primary care provider. They will help to coordinate with specialty care such as cardiology, neurology, orthopedics, podiatry, home care or hospice.
On-site primary care by medical professionals who are skilled and committed to caring for older adults. Preventive care that helps to reduce or eliminate the need for hospitalization. Depending in which senior living location you are at, there are added benefits such as:
- Access to your Medicare day period with no hospital stay, which means no more traditional three-day hospital stay as required under current Medicare plans.
- Access to on-site orthopedic visits for falls prevention and on-site x-rays.
- Fracture management for non-surgical fracture care, surgical referrals
- Arthritis management for chronic, degenerative arthritis
- An M Health Fairview pharmacist is available for medication review, medication therapy management and general questions or concerns about your medications.
Each patient is monitored by an advanced practice provider on a regular basis and whenever there is a change in health condition. Our care team works closely with the nursing staff at each facility to stay informed of any health changes. In Assisted Living, a resident is seen every other month and more frequently if needed. At these regular visits, residents’ care plans and medications are reviewed.
In Long Term Care, all federally mandated visits (every 60 days) are completed by the physician and/or advanced practice provider. Again, our care team is in partnership with the nursing staff and will stay informed of any health changes that may require a visit. Our care team will order necessary tests and medication as needed between visits.
The goal of the onsite care is to provide proactive, preventative care to avoid emergency room visits and hospitalizations. If you should require an emergency room visit or hospitalization, your care plan and medication will be evaluated upon your return for any changes.
Labs and most tests are done on-site by diagnostic and x-ray services. All residents accessing on-site care are required to have medication management services through their senior living community.
M Health Fairview providers work closely with the hospital of patient’s choice. However, we encourage using the M Health Fairview system to ensure continuity of care. During a hospitalization, we will work in coordination with the hospitalist, Transitional Care Unit and nursing homes on transitions to coordinate patient care.
There is no additional cost to be enrolled in Fairview Partners. Coverage for services is based on the health insurance plan you have selected. There are added benefits such as access to your Medicare day period with no hospital stay, rather than the traditional three-day stay that is required under Medicare, with an in-network Medicare Advantage Plan.

Fairview is a Medicare participating provider and agrees to accept Medicare’s allowable charges. We directly bill Medicare and/or your insurance company. Typically, your insurance will fall into one of the following two categories:
Traditional Medicare, Supplemental Insurance and Non-Network Health Plans
M Health Fairview Geriatric Services is a Medicare participating provider and accepts assignment for residents who are unable to change health plans to one our in-network health plan options or plan to switch but are not in Medicare’s annual open enrollment period. M Health Fairview is able to directly bill either Medicare or your current health plan. Note: there are certain benefits that are not available to residents when you are not enrolled in an in-network health plan.
