Coordinated medical and behavioral care

Eligible patients can receive medical and behavioral health care at Fairview Clinics – Hiawatha, Fairview Clinics - Integrated Primary Care, or Fairview Clinics – Andover. This coordinated care team includes a behavioral health provider, social worker and community health worker. Fairview’s Primary Care Behavioral Health Home team works closely together to help you work through day-to-day challenges and move toward healthy living.

Who’s eligible?

The Minnesota Department of Human Services establishes the criteria. Patients must:

  • Be insured through Medical Assistance, PMAP or both Medicare and Medicaid
  • Be diagnosed with serious and persistent mental illness or disturbances

The following services are offered to eligible patients:

  • Comprehensive Transitional Care – When you need Emergency Room or hospital care, we are here to support you as you return home.
  • Individual and Family Support – We’re here to help you when you need it.
  • Referral to community and social support – We can help you locate other helpful services and resources in your community.
