There are no fees to use Fairview MyChart. However – e-visits are part of MyChart, which allow you to have a visit with your provider without a trip to the clinic. Access to the MyChart system is granted to users for the purpose of furthering their medical care. Use of the system unrelated to this purpose may result in the suspension or termination of access privileges. Improper activities include, but are not limited to, as the use of inappropriate or abusive language, reserving appointment times that are not needed or are frequently cancelled and other activities that are deemed to cause a disruption to clinic operations in the sole discretion of the Host Sites.
Other TermsThe Host Sites shall not be liable for any damages suffered by the user or other third parties related to, directly or indirectly, the material and information contained in MyChart. In no circumstances will the Host Sites be liable to the user or any other person for direct, indirect, special, incidental or consequential damages of any type, even if informed of the possibility of such damages. The Host Sites cannot guarantee access to the MyChart system and are not liable for the user's inability to access or view the information contained on the site. By downloading or viewing the information on MyChart, the user agrees to hold the Host Sites harmless and indemnify them against any damages, claims, or other claims asserted against the Host Sites arising out of, or in any way connected with, the user's utilization of MyChart or the information contained therein.
This Agreement shall be governed by the laws of the State of Minnesota and any dispute arising hereunder must be submitted to binding arbitration in Minneapolis, Minnesota, pursuant to the Commercial Arbitration Rules of the American Arbitration Association. MyChart information and the logos, icons and other marks shown or referenced within are reserved by the Host</div> <div>Sites and are protected by copyright and trademark law. The user may not reproduce such information without the prior written consent of the Host Sites. This Agreement contains the entire agreement between the parties with respect to the subject matter contained herein and it shall not be modified or amended except by signed written instrument. The terms of this Agreement may be changed at any time in the discretion of the Host Sites and updated terms will be posted on the MyChart website for user access.
Your clinic uses electronic record systems and believes they are an important part of improving the quality and safety of the care we give. Physicians authorized practitioners and authorized staff use these systems to provide the information needed to treat you. Policies, processes and technical protections are in place to keep your information from being seen by anyone who should not see it. Our policies and processes conform to state and federal law and are designed to safeguard your privacy. Our staff is educated in confidentiality and the appropriate use of medical information.
Your Medical RecordMyChart is an Internet application that enables you to have secured web-based access to portions of your electronic medical record, and allows secure messaging with your care team. Messages you send via MyChart will be made part of your permanent medical record. Once it is made part of your record, it will be accessible to current and future staff members who are involved in your care.
Secure CommunicationAll MyChart communications between you and your clinic health care team are carried over a secure, encrypted connection directly into the electronic medical record. While you may receive Internet e-mail messages notifying you of new messages in your MyChart Inbox, these e-mails will never contain any confidential medical information. All medical record information and MyChart messages reside securely behind information system firewalls.
Access CodesWhen you initially enroll to use MyChart, you will be provided with an access code. It is extremely important that you keep your MyChart access code, login name and password completely confidential from everyone. If your access code and password is available to others, they will be able to view your medical information and communicate with your clinic as if they were you. It is your responsibility to prevent disclosure of your access code and to change your password if you feel your security has been compromised. You can change your password online at any time.
