Employee Occupational Health Services

Depending on your role at Fairview, you may be required to successfully complete a health evaluation with Fairview's Employee Occupational Health Services team prior to starting work at Fairview. Your health evaluation must be completed a minimum of seven (7) working days prior to your start date.

Complete your pre-placement health evaluation

Read the following to successfully and efficiently complete your pre-placement health evaluation prior to your start date. By understanding the requirements and following the steps of a pre-placement health evaluation, you are taking initial steps to a successful career at Fairview.

In-person health evaluations require appointments, take place at multiple Fairview sites and are available Monday-Friday between 8 am-4 pm with the last appointment of the day at 3 pm based upon availability. Refer to available locations listed below.

Pre-placement health evaluations are medical appointments. Come prepared for the appointment (see preparation activities below), allow sufficient time for the appointment and arrive 15 minutes prior to appointment time. Refrain from bringing children or other adults to the appointment as there is no one to watch children or others during your appointment. Your health screening appointment may be rescheduled and your start date may be delayed for late appointment arrivals, if children or others are brought to your appointment or when historical health documents, as noted below, are not provided.

Please note: If you will be working at a Fairview site that does not provide any direct patient care, Employee Occupational Health will contact you by phone for a health review. An in-person appointment may or may not be needed.

Prepare for your appointment

1. Click to print and complete the confidential health screening form prior to your appointment (if you are not able to complete the form in advance, please arrive 15 minutes prior to your scheduled appointment to complete necessary paperwork)

2. Bring historical documentation to your appointment of previous immunizations, blood tests, tuberculosis screenings, current or permanent work restrictions (see below for examples of acceptable documentation)

3. Record of immunizations/proof of immunity for:

  • Measles
  • Mumps
  • Rubella (German Measles)
  • Varicella (Chicken Pox )
  • Hepatitis B
  • TDAP
Acceptable documentation for proof of immunity to Measles, Mumps and Rubella includes:
  • Documentation of 2 live virus vaccines on or after 12 months of age, or
  • Documentation of positive titer
Acceptable documentation for immunization/proof of immunity to Varicella (Chicken Pox) includes:  
  • Documentation of 2 vaccines
  • Documentation of positive titer
  • If age 50+, documentation of Zoster vaccine received at or after age 50
Acceptable documentation for immunization/proof of immunity to Hepatitis B which is recommended for all employees but required only for those may have direct contact with blood and body fluids:
  • Documentation of three vaccine series in a six-month period, or
  • Documentation of positive antibody titer, or
  • Completion of a declination form
Acceptable documentation for TDAP immunization is complete TDAP immunization documentation from your healthcare provider or State immunization registry documentation of the TDAP vaccine

Immunization/proof of immunity records may be obtained from your school, medical provider or previous employer and faxed to the applicable Employee Occupational Health office.

4. Historical record of Tuberculosis screening (see below). Appropriate Tuberculosis screening will be performed at your appointment:

Acceptable historical record of Tuberculosis Screening includes:

  • Documentation of 2-Step Mantoux test
  • Blood assay results for Mycobacterium Tuberculosis test (QGold or TSpot)
  • Documentation of previous positive PPD/BAMT, CXR and documentation of medication therapy

Pre-placement health evaluations are conducted at the following locations. Please refer to your offer communication details for the location and time of your appointment. Should you need to reschedule your pre-placement health evaluation, contact your recruiter.

Notice to new employees: When visiting the Employee Occupational Health Services office(s), please park in the areas outlined in the site directions below to receive free parking validation. We are unable to validate or otherwise compensate parking fees outside of the approved parking locations.

  1. From the hospital entrance, proceed to the second floor via the main staircase or elevator.
  2. Follow the long hallway, almost to the end (see sign pointing to Human Resources).
  3. Take a right just before the cardiac rehab area.
  4. Proceed through the door at the end of the hallway and take another right (follow sign pointing to Human Resources and Employee Health).
  5. Follow hallway to Suite #2188 (sixth door on the left).

  2. From the main entrance, take the short hallway to the right of the information desk toward the elevator.
  3. Take the elevator to the 3rd floor.
  4. Go left off of the elevator and proceed to the end of the hall.
  5. Human Resources is the last door on the right. Call if you get lost.

  2. The HR office will be on your left about halfway down the hallway (look for signage).

  1. Park in the patient/visitor parking ramp on Delaware Ave SE. From parking ramp go down to the street level.
  2. Walk on foot on Delaware Ave and walk across Harvard St. SE. Continue to walk on the sidewalk down Delaware St. SE.
  3. After you pass between the Moos Tower Building and the Phillips Wangensteen Building, the Mayo Building will be in front of you. Enter through the door with the sign over it that reads “Mayo Building.”
  4. Once inside the building, take your first left.
  5. Then, take your next left. The EOHS office (B324) will be at the end of the hallway.
