Providing the community with the tools and knowledge to live a healthier, better life is a goal of Kathy Bystrom, manager of community health outreach at Fairview Lakes and Fairview Northland medical centers.

So when obesity and related chronic diseases were noted as important community needs in our Community Health Needs Assessments (CHNA), it was the perfect time to put Kathy’s ideas into action.

In partnership with the YMCA Forest Lake, Fairview Lakes Medical Center and Fairview Clinics – Hugo created the Well Together program.

Well Together looks at all aspects of an individual’s current health situation—offering courses and consultation about nutrition, healthy eating, mind-body medicine, wellness, health coaching and fitness.

“I am so thrilled to help launch this program to the communities we serve,” says Kathy. “It has all the components needed for someone who would like to make a change and live a healthier life."

Well Together has it all

This comprehensive program takes a holistic approach to improving a participant's overall health and well-being—all conveniently at a neighborhood YMCA.

“The Well Together program is the only one of its kind,” says Kirk Erickson, director of healthy living at the YMCA Forest Lake. “We are able to bring together professionals like physicians and fitness trainers to educate YMCA members who are working towards the goal of living a healthier life." 

Focus on the entire body

Fairview doctors, nurses and dieticians, along with YMCA fitness professionals and wellness experts, work closely with program participants to meet their unique needs.

Participants first complete a whole health assessment—a one-hour consultation with a Fairview physician to create a Personal Health Action Plan. This plan outlines specific and customized, individual health goals.

Then participants work their way through nine core elements, including courses on nutrition consultation, fitness assessment and healthy eating group sessions, using the expertise and knowledge from both organizations. 

“I joined the Y after a recommendation from my doctor to strengthen my core,” says Mary Johnson, Well Together participant. “Then I was introduced to the Well Together program—it was just what I was looking for. All the experts were available to me through the program, including a doctor, nutritionist and a personal trainer."

Ready to make a change

Any member at the YMCA Forest Lake is welcome to participate in the Well Together program. The ideal candidate is between 25-65 years old, has a body mass index (BMI) of 28 or higher and is motivated and ready to make a change.
