We are conducting a survey to learn more about the most pressing health needs people face in the communities we serve. The survey is part of the Community Health Needs Assessment process we undertake every three years,
“This 17-question survey is just one data source we are using as part of our comprehensive Community Health Needs Assessment,” says Ann Ellison, Fairview director of community health and church relations. “This survey will help us assess how best to meet health needs of the community going forward.”

Community health staff will work with community members to reach out to underserved populations. “We want to make sure we capture as broad a swath of the community as possible,” says Ellison.
The survey deadline is Thursday, Oct. 8.
