Your privacy matters

Your privacy and the security of your personal information is important to us. Any facility that uses our electronic health record is subject to state and federal privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA). Everyone who has access to the system is trained to only use the information as allowed by law.

Fairview takes the privacy of our patients’ health and personal information very seriously. We monitor the use of our system to ensure it is being used appropriately.

Fairview Connect

Fairview Connect is our electronic health record system. It allows members of your care team to view your history and record and manage your care, no matter which Fairview location you visit. See a list of entities within Fairview who are subject to the Joint Notice of Privacy Practices and who may use the Fairview electronic health record. In addition to Fairview hospitals and clinics and related entities, the following providers use this system:

  • Richfield Medical Group

How it works

Our electronic health record centralizes your health information in one place so that it is available for your care team to provide personalized treatment whenever and wherever you seek care.  

With Fairview Connect, your medical care and history are all kept in one place and follow you  whenever you visit our hospitals or clinics. This helps caregivers avoid duplicating services and decreases the risk of errors. Providers can also easily monitor any ongoing health issues you have. 

A single electronic health record also saves time, because you don’t have to fill out the same forms and answer the same questions at each visit.

How to make changes

When you visit us, you will be asked to consent to use of a shared electronic health record that is used within Fairview and its related entities as well as at Fairview Connect affiliates. If you decline to provide consent, the availability of your record will be restricted to only Fairview’s entities. Your consent applies to any current, past and future health care records in our system. If you choose not to consent, you can still get care at our hospitals and clinics. We will never turn you away.

If you have concerns regarding the privacy of your health information or wish to request restrictions on certain uses and disclosures of your health information, you may contact the Fairview Privacy Office as noted in the Notice of Privacy Practices

Care Everywhere

Care everywhere is part of an electronic health information exchange (HIE) that allows participating healthcare organizations to electronically exchange your patient health information during and as a part of your care and treatment.

How does the exchange work?
The first time you present to one of our care systems, you will be asked for consent to participate in Electronic Health Information Exchange (HIE) on our Consent for Service form. This consent is provided at your first visit and valid for 5 years. You have the choice to agree or decline the option of HIE. If you decide to decline or “opt-out,” your health information will not be available for electronic exchange or sharing except in cases of emergency or where required by law. This decision only applies to the HIE and does not include standard healthcare uses and disclosures for treatment, payment and operations.

What type of information is shared?
Patient health information includes any information in your record that is used for treatment, including:

  • Allergies
  • Medications
  • Medical history
  • Immunizations
  • Patient visit summaries
  • Test results
  • Diagnosis

How does Care Everywhere benefit me?
When you visit a non-Fairview Clinic or Hospital, Fairview will be able to electronically exchange information about the tests, procedures and other information gathered during your visit. Exchanging health information between medical organizations has traditionally been a time consuming process.   Should you choose to consent to the HIE, your care team can access information from compatible outside organizations in a matter of minutes. This will help your health care team efficiently and safely meet your health care needs.

Who can view my information?
Only the health care professionals involved in your care are able to view your information.  If you have declined or “opted out” of HIE, your care provider may ask for your consent to obtain your records.

How is my privacy protected in Care Everywhere?
All participating organizations are regulated by state and federal laws and use a combination of technical and administrative safeguards to protect your health information. Usage of the system is monitored to ensure privacy protection.

What is the risk to Care Everywhere?
The only added risk is your health record is that care teams involved in your care will now see your health information through an electronic connection rather than by paper (mail or fax).  All health care professionals are already responsible for keeping your health records private. Should you choose not to participate in HIE, this will not affect your ability to receive care at our sites.
