Pricing/cost questions

• Service estimates and payment: Fairview will provide a good faith estimate of what we expect to charge you for medical care at a Fairview hospital or clinic. In order to provide you with the correct estimate, please have available the five-digit Current Procedural Terminology Code (CPT Code) associated with your procedure.  If you do not know the specific code(s) you can get them from your doctor’s office.  Whether or not you have insurance, call the hospital or clinic where you will be going and give the CPT Code that identifies what service you need to obtain an estimate. • Paying without insurance: If you don’t have insurance, ask to speak with the registration department or patient financial counselor at the hospital or clinic. He or she will discuss payment options with you.
• Paying with insurance: What your insurance plan pays for a procedure depends on your coverage. Check with your plan administrator to verify coverage. We do have information about what various insurance companies will pay Fairview for a given procedure. Call our plan reimbursement line, 612-672-1048, and be ready with CPT codes, the name of your health plan, your member number, where your procedure will be performed and if it is inpatient (staying overnight in the hospital) or outpatient (no overnight stay in the hospital).
