Palliative Care Staff

The Transitions and Life Choices team consists of clinical staff and administrative staff who provide leadership support for the TLC program and consult teams.

Palliative Care/TLC Consult Teams

University of Minnesota Medical Center, Fairview

The Palliative Consult Service provides coordinated care to address physical, psychological, social and spiritual needs. Members of the consult service include:

Sandra Gordon-Kolb, MD

 

Medical Director, Palliative Medicine

HPM Fellowship Program Director

   

Board certified in hospice and palliative medicine and internal medicine, Dr. Gordon-Kolb serves as medical director for the palliative consult service at the University of Minnesota Medical Center, Fairview. She holds a joint appointment in the Department of Medicine and Family Medicine and Community Health. She is the HPM Fellowship Program Director with the University of Minnesota Academic Health Center. Dr. Gordon-Kolb sees patients at the University of Minnesota Medical Center, Fairview and the Palliative Care Clinic.


   
   

Rudolph Keimowitz, MD

 

Physician, Palliative Medicine

 

Liaison with Department of Hematology/Oncology & Transplantation

 
   

With over 30 years experience as a clinical hematologist and currently a part-time clinical assistant professor of hematology, Dr. Keimowitz sees patients at the University of Minnesota Medical Center, Fairview. He completed his residency in Internal Medicine and a fellowship in Hematology at Boston City Hospital in Boston, Massachusetts.


   
   

Joel Carter, MD

 

Physician, Palliative Medicine

 
   

Board certified in hospice and palliative medicine and family practice, Dr. Carter also has 13 years experience in emergency medicine. He sees patients at the University of Minnesota Medical Center, Fairview and teaches at the University of Minnesota Academic Health Center. He also sees patients at the Fairview Palliative Care Clinic. He was awarded a Bush Medical Fellowship in 2002 to pursue interests in end of life medicine and physician leadership. He completed his pain and palliative medicine fellowship at the Dana Farber Cancer Institute – Harvard University, Cambridge, Massachusetts.


   
   

Bronwyn Long, DNP, RN, ACHPN

 

Clinical Nurse Specialist

 
   

Ms. Long received her Doctor of Nursing Practice degree from the University of Colorado at Denver. As the palliative clinical nurse specialist, she addresses the physical, emotional, social and spiritual needs of seriously ill patients. She works closely with nursing staff, developing educational programming in palliative care.

 
   
   

Dot Landis, MSW, LICSW

 

Clinical Social Worker

 
   

In addition to seeing patients in the Palliative Care Center, Dot is the clinical social worker for the inpatient Palliative Consult Service at the University of Minnesota Medical Center, Fairview. She obtained her masters degree in social work from Rutgers University. She holds a state of Minnesota Licensed Independent Clinical Social Work license and is trained in clinical hypnosis. She is a former instructor at the University of St. Thomas/College of St. Catherine School of Social Work.


   
   

Paul Galchutt, MDiv

 

Chaplain

 
   

With 12 years experience in pastoral care, Rev. Galchutt joined Fairview in 2007. As a member of the palliative consult service and a staff chaplain at the University of Minnesota Medical Center, Fairview, Rev. Galchutt offers spiritual and emotional support, counseling, and coping strategies for patients and families according to faith and individual needs. He received his Masters of Divinity degree at Luther Seminary in St. Paul, Minnesota.


University of Minnesota Children’s Hospital, Fairview

Julie Hauer, MD

 

Physician, Pediatric Palliative Medicine

 
   

After completing a fellowship in pediatric palliative care,
Dr. Hauer returned to the University of Minnesota Children’s
Hospital as a consultant in pediatric palliative care. Dr.
Hauer has a special interest in children and neurological
conditions and is board certified in general pediatrics and
board eligible in palliative care.


   

Fairview Ridges Hospital

Barbara St. Marie, MA, ANP, GNP, PhD (c)

 

Nurse Practitioner

 
   

Certified as an Adult and Gerontology Nurse Practitioner, Ms. St. Marie has specialized in pain management and hospice care for 27 years. Her focus is caring for patients with life limiting diseases at Fairview Ridges Hospital in Burnsville, MN.

 

Fairview Red Wing Medical Center

Jessica Reese, MSN, APRN, BC, FNP

 

Clinical Nurse Specialist

 
   

as a family nurse practitioner, Ms. Reese offers palliative care consultation for Fairview Red Wing Care System.
