To register for a Continuing Education course, please submit the following information:

1.  External CE Contract: The External CE Contract form is to be filled out for attending any external continuing education course that is not sponsored by Fairview.

2.  Copy of course brochure.

3.  Registration form completely filled out (except payment information).

Send all of this information to Shawna Freeman once everything is completely filled out and all signatures are obtained. Please refer to the CE Guidelines for more information.
