Requirements before starting the internship
1. A minimum of a baccalaureate degree from an accredited college/university
2. An overall minimum GPA of 3.0 from the baccalaureate program
3. Completion of DPD requirements from an accredited college/university
4. Meeting the “recency of education” requirements [DPD completion within three years of applying to the internship. If DPD was completed > three years prior to internship application, make up courses will be required. Please contact Internship Director for further information]
5. Documentation for the following immunizations

MMR (measles, mumps and rubella): Documentation of two vaccinations is required, or a positive history of the disease is sufficient if born prior to 1957, or documentation of positive titers.

Varicella (chickenpox): Documentation of two vaccinations is required, orr documentation of a positive history of the disease, or documentation of positive titers.

A negative TST test (Tuberculosis Skin Test or Mantoux): Interns must have a negative 2-step TST test in their medical history with annual negative TST checkups. Interns with a positive TST test must have a negative chest x-ray test within the 12 months prior to the start of the program.

Hepatitis B Vaccination Series: A 3-shot Hepatitis B vaccination series is required.
6. The following immunizations are recommended, but not required.

Pertussis (t-dap) Vaccination: One-time vaccination after age of 19.

Annual Flu Shots (influenza): Annual flu shots are highly recommended.
7. ADA membership
8. Serv safe certification. With prior authorization, interns may be allowed to complete certification during the internship. Serv Safe certification is mandatory for graduation.
Requirements during the internship
1. Compliance with policies set up for the program
2. Completion of all rotations and projects
3. Meeting the physical and mental demands outlined in the tables below.
