2011 University of Minnesota Medical Center, Fairview Medical Staff Recognition Awards
Nominate an active member of the credentialed medical staff. Each winner receives a cash award of $5,000 from the Medical Staff, presented at the Medical Staff Awards Dinner on Oct. 4. Award prizes are intended to support some aspect of the recipient's work.

Any member of the Fairview or Academic Health Center community, including University of Minnesota Physicians, may nominate. Self-nominations are accepted. Some aspect of activities leading to the nomination must have occurred in 2010.
Award Criteria
Clinical Care Award Criteria
Provides exemplary care to patients in the spirit of Fairview’s mission of improving the health of the communities we serve
Examples:
• Receives consistently high ranking in patient satisfaction data
• Demonstrates caring, compassion, competency and good communication skills
• Demonstrates superior patient care outcomes
• Held in high esteem by colleagues and staff. Someone you would refer a friend or family member to.
Fosters an environment of collaboration and collegiality and teamwork
Examples:
• Receives consistently high ratings in feedback from colleagues and team members
• Serves as a mentor to new medical staff
• Supervises or mentors residents or medical students
• Has served on one or more medical staff committees in the last year
• Is committed to human service through volunteerism
Demonstrates deep understanding and respect for patients by modeling the Fairview values of dignity, integrity, service and compassion
Examples:
• Has served on medical mission
• Offers medical inservices to colleagues or physicians in training
• Promotes Fairview services in the community through participation in news media interviews or community presentations.
Clinical Innovation Award Criteria
Demonstrates success in redefining the delivery of patient care in the spirit of Fairview’s mission to improve the health of the communities we serve
Examples:
• Has initiated or participated in a hospital quality improvement process improvement effort, making something more patient or family friendly
• Has set up a hospital/clinic based clinical program
• Can demonstrate improved outcomes in at least one aspect of the Institute of Medicine’s Six Aims of Care:
- Safe—avoiding injury from the care that is intended to help the patient
- Effective—based on the use of systematically acquired evidence to determine whether an intervention, such as a preventive service, diagnostic test, or therapy, produces better outcomes than alternatives - including the alternative of doing nothing
- Patient centered—respectful and responsive to individual patient preferences, needs and values, and ensures that patient values guide all clinical decisions
- Timely—avoiding waits and sometimes harmful delays for both those who receive and those who give care
- Efficient—avoiding waste, including waste of equipment, supplies, ideas and energy
- Equitable—does not vary in quality because of the patient’s individual characteristics such as gender, age, race or ethnicity, sexual orientation, spiritual beliefs, primary language, geographic location, physical capability or socioeconomic status
Clinical Scholar/Research Award Criteria
• Demonstrates outstanding accomplishment through national or international recognition in patient-based scholarship and research
Nominations Must Include:
• Written narrative of not more than 1500 words
• Letters of support from three clinical associates. For clinical scholar award, one name must be from outside Fairview or the University of Minnesota.
• If applicable, relevant supporting documents, which may include news media articles, committee reports, bibliography, patient satisfaction data or related material.
