Dear patients and families,

Fairview's ability to accept Beech Street insurance at any Fairview clinic (including the former Columbia Park Medical Group sites) will end on Jan. 31, 2009.

Due to this change, you will need to make some decisions about your health care. We care about you and hope you can continue to be your health care provider. However, we realize that choosing an alternate clinic may be necessary if health care coverage provided to you through Beech Street is your only insurance option.

If you would like to explore options for how to retain Fairview as your health care provider of choice, please contact our Central Business Office at 612-672-5390.

We encourage you to contact Beech Street directly to identify clinics and health care providers available to you. You can find their contact information on your insurance card. You can request a Transfer of Medical Record form, should you choose to transfer your care to a new clinic that continues to contract with Beech Street.
