Columbia Park Medical Group and Fairview
Frequently Asked Questions

Last January Columbia Park Medical Group (CPMG) and Fairview Health Services merged to enhance health care in the northern suburbs of the Twin Cities.

1. What changes will I see?

  • Most noticeably, the name ‘Columbia Park Medical Group’ will be replaced with the name ‘Fairview’ in all advertising, media, online and printed materials including exterior signage at clinic sites.
  • An electronic medical record will be implemented at all clinic sites later in 2009 which will enhance and improve services for patients. Providers will be able to access any existing patient’s medical record on-site within minutes.
  • My Chart, Fairview’s online medical chart, will be available to CPMG patients late next year, allowing patients to schedule appointments online, view test results, request a prescription refill, email a question to a provider and even conduct an E-visit with a provider.
  • CPMG’s website, http://www.cpmg.org/, will be replaced with http://www.fairview.org/cpmg/ on January 1, 2009.

2. Will my care change? No. You will still see your same provider, the same nurses and the same support staff at your regular clinic. Your relationship with your care team will not change.

3. Will my clinic close? No. Your clinic will not close. All clinics are staying right in your neighborhood, where we have always been.

4. Can I still go to Unity Hospital? Yes. Although patients may still be referred to Unity Hospital in Fridley, they will now also benefit from enhanced access to the continuum of care at any Fairview hospital or facility, including University of Minnesota Medical Center, Fairview and University of Minnesota Children’s Hospital, Fairview. Feel free to discuss these new options with your provider.

5. Will my clinic still accept my insurance plan? Yes. We will continue to accept nearly all current insurance plans. Patients should contact their employers or insurance companies with questions about coverage and reimbursement.

6. Will there be any changes to the pharmacies? No. Fairview Pharmacy will continue to operate pharmacies in the Andover and Columbia Heights clinics, as well as the newly opened pharmacy in the Brooklyn Park Clinic. With the implementation of an electronic medical record, providers will be able to send prescription orders to the pharmacy during the patient’s appointment, for them to pick up before leaving the clinic.

7. Who is Fairview Health Services? Headquartered in Minneapolis, Fairview is a not-for-profit integrated healthcare network nationally recognized as a leader in clinical excellence and innovation. Fairview’s continuum of services brings continuity of care to patients through seven hospitals, including the University of Minnesota Medical Center, Fairview and University of Minnesota Children’s Hospital, Fairview, more than 30 primary clinics and 55 specialty clinics, home care and hospice services, rehabilitation services, inpatient and retail pharmacies and senior services including long-term care and various levels of housing, as well as adult and intergenerational daycare programs.
