We’re changing health care
Fairview has been on a journey, since 2008, to change how we deliver health care.

We’ve been developing our capabilities to manage the health of populations of patients over time. Our focus is on delivering greater value. That means:
  • improved health (better outcomes and better quality of life for people with chronic health conditions)
  • improved care (exceptional patient safety and experience)
  • lower total cost (more affordable care)
A new model for care
The United States spends a lot on health care and costs have been growing rapidly. Yet, as a nation, we don’t have great health outcomes to show for what we spend.

To address this problem, Fairview has been pushing to be held accountable for the care we provide. Why? Because we believe we provide greater value for our patients.

Fairview has various public and private payer contracts that are based on our performance and the value we provide, rather than the traditional fee-for-service. We’ve been partnering with public and private payers to serve particular populations through the Fairview Health Network.

A population can be any group of people—such as:
  • employees or individuals who choose one of the commercially-available Fairview Health Network insurance plans on their own or through their employer.
  • patients covered by Medicare who have selected Fairview as their provider and who are part of our groundbreaking work as one of 32 Pioneer Accountable Care Organizations nationwide.
More planned, coordinated care
We have made many changes to our care delivery to ensure our patients get the care they need, when they need it. This is especially important for patients with chronic health conditions.

Fairview has redesigned the way care is delivered in our 40+ primary care clinics by:
  • organizing staff into care teams to provide a more coordinated, seamless patient experience. Each team includes a physician, nurse practitioner, nurses, medical assistants and schedulers.
  • engaging patients more directly in their care and in the management of their health.
  • using collections of clinical best practices—known as care packages—for treating many common diseases or conditions including low-back pain, diabetes, hypertension, migraine, prenatal care and more. These help ensure that patients get the best, most consistent care possible.
Offering new ways to receive care—including nurse-only visits, virtual care visits via email, phone, online and group appointments.

Using new technology
Fairview has been using new technology to improve patient care and experience while reducing costs. For example:
  • All of Fairview’s hospitals and clinics are on a unified electronic health care record. This helps us more effectively manage care delivery and patient transitions across all our different care settings.
  • Beyond Fairview, we’ve been active in helping develop Minnesota Health Information Exchange, a statewide secure electronic network to share data among providers and between providers and payers.
