What is the Memory Loss Assessment Clinic?
The Memory Loss Assessment Clinic works with people suffering from all types of memory loss: from the early stages of memory loss to patients who have identified memory loss, such as Alzheimer's disease or vascular dementia.
It is important to have an assessment done at the memory loss clinic when a person is suffering from one or all of the following:
- Difficulty thinking or remembering
- Changes in personality or behavior (increased aggressiveness)
- Loss of ability to function independently
- Increasing disorientation to time and place
Once the need for an initial assessment has been identified, the following occurs:
Initial memory loss assessments are 60 minutes in length and include a detailed medical history, laboratory testing and cognitive (thinking, reasoning and remembering) testing. These tests help determine if a patient has symptoms that indicate dementia, and if so, the patient's degree of impairment.
After the initial assessment and testing is finished, our providers create a plan of care that is individually tailored to each patient. Afterward, our case manager works with the patient and the family to help with the plan of care.
Do I need a referral to make an appointment at the Memory Loss Assessment Clinic?
Although a referral from your primary physician is the preferred way to have an initial appointment scheduled, it is not required.
When I start going to the Memory Loss Assessment Clinic, does it replace my primary care clinic?
No, the Memory Loss Assessment Clinic does not replace a patient's primary care clinic; memory loss assessments augment the patients' care.
What insurance does the Memory Loss Assessment Clinic accept?
The Memory Loss Assessment Clinic is a participating provider for most local health plans and major insurance plans as well as Medicare and Medicaid.
As a family member will I have the opportunity to speak to the physician to voice my concerns about my loved one with memory loss?
Yes, family members/caregivers are a big help in filling in the gaps and getting a better understanding of the degree of memory loss. If your loved one agrees during the appointment, time will be made; otherwise, before the appointment, you can speak to the nurse case manager about your concerns and she will relay this information to the physician.
Will my primary care physician get a copy of what went on during the visit?
Yes, if you are in the Fairview system, files are maintained through our electronic health record system; otherwise, your health care record will be mailed or faxed to your primary care clinic.
What can I expect out of the appointment at the Memory Loss Assessment Clinic?
The physician and nurse case manager will meet with you and your family/caregiver (if you bring them to the appointment) all together. Then the physician will meet with the family/caregiver, if the patient agrees, and the nurse case manager will perform screening tests with the patient. Once tests are completed, everyone will meet together to discuss the next plan, whether that be more lab tests, imaging, medication or more cognitive testing and then follow up to discuss the diagnosis.
