Home care rehabilitation services help you reach your goal of living at home with dignity, comfort and independence. We are available when you or a family member is released from a hospital or long-term care facility and still needs help recovering from an illness or surgery.

Therapists work as a team with you, your caregivers, family and physician to help you reach your highest level of function after an injury or illness and to help ensure your safety at home. They’ll develop a care plan specific to your individual needs. They’ll recommend equipment that may help you and teach you how to properly use and care for it.
