Rehabilitation diagnoses
Some of the medical conditions that may be appropriate for referral to inpatient acute rehabilitation include:
- Stroke
- Hip fracture
- Spinal cord injury
- Brain injury – traumatic and non-traumatic
- Complex medical:
- Transplant
- Cancer
- Polyarthritis
- Congenital deformity
- Amputation
- Major multiple trauma
- Other neurological disorders (i.e. MS, movement disorders, Parkinson’s)
- Bilateral joint replacements
- Single joint, plus:
- age 85+
- Body Mass Index (BMI) 50+
- Complex medical
Must have needs in the following areas during stay:
- Comprehensive rehabilitation
- Rehabilitation nursing
- Two or more therapies (physical therapy, occupational therapy, speech-language therapy)
- Two or more of the following areas:
- Activities of daily living, independent activities of daily living, home/lifestyle modification
- Bed mobility/transfers
- Speech and language retraining, swallowing
- Cognitive retraining
- Range of motion, strengthening
- Positioning, splinting
- Wheelchair mobility, gait, balance
Medical management
Needs medical care for one or more of the following:
- Skin care
- Medications
- Bowel and bladder issues
- Respiratory issues
- Pain or spasm
- Nutritional issues
- Coping skills
- Adjustment to loss
