Individual Investigator Agreement (IIA)

 

External researchers interested in conducting research with Fairview Health Services are required to provide Fairview Research Administration with a signed Individual Investigator Agreement form This form provides written assurance to Fairview from external investigators that they agree to comply with federal, state and institutional (Fairview’s) regulations and policies for research involving human subjects.
 
Researchers are considered “external” if they are not
  • an employee of Fairview Health Services,
  • a University of Minnesota Physicians’ physician, or
  • a faculty member or student of the Academic Health Center at the University of Minnesota, with the exception of adjunct appointments.
