Conflict of Interest Disclosure (COI)
Fairview and community researchers conducting research within Fairview must work with FRA to disclose any possible conflicts of interest. This specifically applies to research sponsored by business or industry, or that involves pharmaceuticals, biologics, or devices.
COI forms should be completed before initially submitting a study to the IRB, and then annually at the time of Continuing Renewal. The form with the original signature must be mailed to FRA.
