Fairview Pharmacy offers $4.99 prescriptions for a 30 day supply of listed generic drugs and $12.99 prescriptions for up to a 90 days supply for the same listed drugs.  Not all generic prescriptions are included in this listing. This listing is subject to change. Fairview reserves the right to modify or discontinue the listing or modify the terms and conditions that apply to the listing at any time without prior notice.

$4.99 prescriptions include up to a 30-day supply of covered drugs at commonly prescribed quantities (as listed on program listing). $12.99 prescriptions include up to a 90-day supply of covered drugs at commonly prescribed dosages (as listed on program listing). Physician permission may be required to change a 30-day prescription to a 90-day prescription.

To qualify, prescriptions must be filled at a Fairview Pharmacy retail location. New prescriptions must be picked up at a retail pharmacy location. Refills may be ordered on-line, via telephone, or in person and must be picked up at a retail location.
