Fairview's Medication Therapy Management (MTM), in partnership with the Metropolitan Area Agency on Aging, provides home visits by a specially trained pharmacist to increase understanding and help individuals get the most out of the medicines they are taking.
MTM is free to qualified senior citizens throughout 2007 under a program funded by the Metropolitan Area Agency on Aging as part of the Older Americans act (donations are accepted but not required).

Do I qualify for free MTM?
You may be eligible for free MTM service if you:
- Are 60 years old or older
- Take four or more medications, whether prescriptions or over-the-counter drugs
- Are homebound (due to illness, disability or functional need) or are a resident of an independent senior living or assisted living facility
- Live in Anoka, Carver, Dakota, Hennepin, Ramsey, Scott or Washington counties
