What is a care team?


A care team is all the people who work together to help you reach your health goals. From your doctors, nurses and other providers to all the health care professionals at Fairview Home Infusion, all are a part of your care team.

Our team of registered nurses, pharmacists, dietitians, pharmacy technicians and insurance specialists work closely with your doctors to provide you with personalized infusion medicine therapy and develop a home care plan that works for you and gives the best results.

From the moment your provider contacts us, the Fairview Home Infusion care team begins work to:

  • Verify your insurance and advocate on your behalf.
  • Process infusion medicine orders sent to our pharmacy.
  • Deliver your infusion medicines and supplies to your home.
  • Schedule one of our nurses to visit you at home to explain your infusion therapy, help you administer your first dose of medicine and answer any questions you might have.

After your therapy begins, a nurse will visit you regularly to assess your progress, change your catheter dressing, draw samples for lab tests and talk through any questions you might have.

Our pharmacists and nurses are available 24 hours a day, seven days a week to answer your questions.
