Medication Reconciliation at Fairview

Medication reconciliation means double-checking your medicines when you enter or leave a hospital or are treated at a clinic. Such double checking is key to reducing medication harm.

Fairview’s current range for perfect medication reconciliation extends from 30 percent to 90 percent. At 90 percent, Fairview Lakes Medical Center has been recognized as a national leader. See the charts below for Fairview's current results. Although there are no national standards, our goal is to improve our own results each year.

Measuring medication reconciliation includes:

  • Looking at a sample of 20-30 patients at each hospital or clinic.
  • Reading a hospital patient's chart to determine if medication reconciliation was done during all three phases—admission, discharge and transfer.
  • Reading a clinic patient's chart to determine if medication reconciliation was done during the visit.
