Home care services we offer
- Skilled nurses are available on call 24 hours a day, every day
- Nursing procedures such as wound care, dressing changes and IV therapy
- Medication management
- Physical, occupational, lymphedema and speech therapy
- Social work services: resource coordination, patient and family support
- Assistance with daily activities such as bathing, dressing, mobility and homemaking services
- Physical, emotional, social and spiritual care
- Health care education to help you and your family manage your illness (e.g. diabetes, respiratory/cardiac disorder)
- Telehealth monitoring: daily home monitoring of patients' clinical trends such as blood pressure, heart rate, weight, etc.)
