At Fairview Home Care and Hospice and Fairview Lifeline, our mission comes first...to serve each person with dignity, integrity, service and compassion. Because home care and hospice payment often falls short of the actual cost of services, we rely on financial gifts from generous donors to meet our patient and staff needs. Hospice donations and memorials provide benefits such as:
- Financial assistance for patients without resources to pay for needed services
- Music and massage therapy to ease anxiety and pain
- Patient and family education materials such as books, pamphlets and newsletters
- Medication delivery when needed
- Bereavement care for family and friends
- Training and support for hospice staff and volunteers
Home care donations and memorials provide benefits such as:
- Financial assistance for patients without resources to pay for needed services
- Visits from the home care chaplain for spiritual comfort and support
- Counseling and spiritual care visits for patients with an advanced, progressive illness
- Resources such as clinical reference manuals and resuscitation manikins
- Cell phone airtime for home care staff to improve rapid communication and enhance their security
Lifeline donations and memorials are used to support:
the sliding-fee scale that allows clients at all income levels to receive Lifeline services.
All contributions are received with gratitude
Donations and memorials totaling $500 or more are acknowledged on a public display wall in the lobbies of Fairview Southdale Hospital and Fairview Northland Regional Medical Center. Levels of giving:
- Dignity $500-999
- Integrity $1,000-4,999
- Service $5,000-$9999
- Compassion $10,000 and above
Gifts may be given in the form of memorials for loved ones who have died, tributes to staff or volunteers who helped make a difference, bequests and estate gifts or general donations in support of our mission. Donations may be made on-line at www.fairview.org/foundation
