Heart Disease Risk - Health Education

Take our online assessment

An online Heart Health Assessment offered by University of Minnesota Physicians Heart can help you determine your risk for developing heart disease within the next 10 years and provide you with suggestions to reduce your risks.

Part of preventing heart disease is knowing whether you’re at risk of acquiring the disorder and then taking action to prevent the onset of the condition.

The team at the Heart Clinic at University of Minnesota Physicians Heart is prepared to get you started on the right path to a healthy heart. Our cardiologists and other Heart Clinic staff take time to listen to your concerns and questions, and to partner with you in the prevention and/or treatment of heart disease.

  • High LDL, or “bad” cholesterol and low HDL, or “good” cholesterol
  • Uncontrolled hypertension (high blood pressure)
  • Uncontrolled diabetes
  • High C-reactive protein
