20th Anniversary this fall with a dinner on Monday,

Guidelines
Special note: Only one grant per applicant per calendar year.
Requirements to partipate in a medical mission:

Be a current Fairview employee with one year of Fairview employment
and at least 20 hours per week, a current Fairview volunteer with a
minimum of 50 hours per year, a Fairview retiree, or a Fairview Physician
Associates (FPA) member.

Are an active participant in a group providing medical treatment,
constructing a medical facility or providing medical education programs.
(Personal education, surveys, religious activity or other non-medical
activities are not eligible for Fairview medical mission funds.)

Provide a brief narrative prior to the trip, stating the goal of the mission
team and the participant's personal goals for the mission.

Submit application by the appropriate application deadline date below.

Complete the trip evaluation within 30 days of return. Use the medical
mission evaluation form below. Failure to provide this information will
make you ineligible for future funding.

Provide documentation of expenses: copies of airline ticket receipt,
medical supplies purchase receipt. Failure to provide this information
will make you ineligible for future funding.
Because medical missions funds are limited, one or more of the following
restrictions may be applied in order to distribute available funds:
1. Individuals who have received Fairview funding fewer than three (3)
times will receive priority.
2. Individuals in jobs below the manager level will receive funding priority.
3. Maximum of three (3) people on same medical mission trip may receive
travel funding.
4. Maximum of two (2) people on same medical mission trip may receive
medical supply funding.
5. One grant per applicant per calendar year.
Please note: Due to the number of applicants and the total funding available
at any time, grants may be approved for less than the requested funding.
Fairview Foundation Medical Mission Committee encourages applicants to seek
funding from a variety of sources (church, family, friends.)
