Fairview Partners has brought back the house call for seniors living at participating campus locations and for those seniors living in their own homes by tailoring primary care needs to each member and providing on-site care and support by our providers.
Fairview Partners is a not-for-profit organization that has been providing care to seniors since 1996 and was one of only 10 programs in the nation to receive a prestigious Robert Wood Johnson Foundation grant for innovative health system reform.
With our program in long term care centers, assisted living sites and available to seniors living independently in their own homes, you will receive care tailored to your needs. You will have a primary care provider, a Fairview doctor, and for some, a geriatric nurse practitioner that may visit you right in your own home or will see you at your primary care clinic, depending upon your needs. Plus, you may also have access to a care coordinator who works to ensure that you are receiving the services you need to continue living independently.
