Electronic Health Record

Insurance Changes

In June of 2008, patients with Humana insurance were notified that effective January 1, 2009, Columbia Park Medical Group would no longer be able to accept any Humana insurance products in its clinics. However, our transition to Fairview Health Services has allowed us to maintain our Humana contract. There has been no interruption in this contract and we can continue to care for you and your family at any one of our clinic locations.

Fairview's ability to accept Beech Street insurance at any Fairview clinic (including the former Columbia Park Medical Group sites) will end on Jan. 31, 2009. Click here for more information.

Fairview Clinics

Fairview Walk-in Clinic

Walk-in clinic locations are at Fariview Andover Clinic and Fairview Brooklyn Park Clinic. No appointment needed. Hours are Monday-Friday, 11 a.m. to 9 p.m. and Saturday-Sunday, 9 a.m. to 5 p.m.
