Fairview treats all patients with dignity and respect, whether or not they are able to pay. We have two programs to assist patients with payment: Hospital Community Care Program and Uninsured Hospital Patient Discount. Both programs support our mission to give quality medical care to everyone in our community.
Hospital Community Care
You may qualify for the Hospital Community Care Program. Your income must be at or below 275 percent of the Federal Poverty Guidelines (see chart below). And you must comply with the terms of the program when you apply. If you qualify, 100 percent of your bill will be paid. If you live out of state and qualify, 50 percent of your bill will be paid.

First you must apply for help from the county. Then you must provide your county approval or denial in writing to Fairview. We will determine if you qualify for Community Care.
We also ask you to follow the rules set by your insurance plan.
Services covered
The Hospital Community Care Program covers charges for most Fairview services. It does not cover charges for:

care that is not needed (for example, care not approved by a Fairview doctor or trial treatments),
-
care that we do not offer at Fairview,
-
services given at Fairview by independent providers,
-
services not billed by Fairview , and
-
services related to transplant.
The Hospital Community Care Program applies only to Fairview's hospital-based care. It does not include care received in free-standing clinics. This is covered under the Fairview Clinic Community Care Program.

To apply for Community Care, fill out our Community Care Application Form. Then mail it to the address on the form.
Uninsured Hospital Patient Discount
Fairview provides an upfront discount to uninsured patients for hospital and hospital-based clinic services that are medically necessary. The discount will show up on your bill. To qualify for the discount:

patient or account guarantor must be a Minnesota resident ,
-
patient must have no insurance,
-
services must be medically necessary, and
-
Fairview must provide the services.
Patients who receive the uninsured discount also may apply for Fairview's Hospital Community Care Program (see above).
