Bridges to Excellence awards Fairview clinics for diabetes, vascular care

A health care coalition has named 23 Fairview clinics as among the best places in Minnesota to receive diabetes or "optimal vascular care." Bridges to Excellence (BTE) is a multi-state, multi-employer coalition developed by employers, physicians and other industry experts. Those recognized include Fairview RidgeValley Clinic for diabetes care and vascular care, Fairview Rogers Clinic for vascular care, Fairview Ridges Clinic for diabetes management and Fairview Oxboro Clinic for diabetes care. 

BTE bases its designations on results of audits conducted

by Minnesota Community Measurement (MNCM), a nonprofit group working to improve health care in the state. MNCM included measurements of 301 sites for diabetes and 272 sites for vascular care.

Overall, all Fairview clinics surpassed the Minnesota diabetes average—17 percent—of patients in perfect control on a combination of five diabetes measures.
