Fairview clinical and administrative leaders have approved a system policy for patient identification. The policy specifies preferred identifiers as patient first and last names and date of birth. If preferred identifiers are not available, acceptable identifiers include medical record number, Social Security number or current street address. Patient room or bed numbers are not acceptable
