When it comes to decisions about your health care, it’s important that your voice is heard. You may not always be able to speak for yourself.
We encourage you to have discussions with your loved ones, cultural or spiritual leaders and health care providers about your goals, values, beliefs and choices.
We are a part of Honoring Choices Minnesota®, supporting and promoting the benefits of advance care planning conversations.
Our goals are to:
- Help you make informed decisions about your healthcare choices and ensure that those choices are honored
- Offer advance care planning discussions with trained staff
- Ensure your choices are clearly defined, documented and available in your medical record
- Translate your choices into medical orders as needed
Why is Advance Care Planning important?
- Know what your health care choices are and decide what is right for you
- An unexpected illness or injury could leave you unable to participate in important treatment decisions
- When choices are left to others to decide that responsibility can be difficult and stressful
- By discussing and outlining your choices, your voice is heard in the care you want to receive
How can I learn more?
We offer free classes at multiple locations, days and times. Our trained facilitators will provide information and guide you through a Health Care Directive document. They can also review, notarize and add your document to your medical record.

How do I document my choices?
There are now two options to share your treatment choices and name your health care agent. Download for free the Honoring Choices Health Care Directive or the one-page version, Honoring Choices Health Care Directive Short Form. Use the document that best fits your needs. If you are a resident of Wisconsin we recommend the Honoring Choices Wisconsin Advance Directive.
