Get help with bills for home care and hospice services

The Fairview Home Care and Hospice Community Care program helps to cover costs associated with home care and hospice services. This program is part of Fairview's mission to provide quality medical services to everyone in our community. We treat all patients with dignity and respect, regardless of their ability to pay.

If you do not have insurance or if your insurance does not cover all your costs, you can apply for help through our community care program. Whether you are eligible for the community care program will depend on several things, including your income (see the chart below). If you have any concerns about your bill or cannot afford to pay, please contact us.

Services not covered
Fairview Home Care and Hospice Community Care covers charges for most of our services. However, it does not cover charges for:

  • products or services that are not medically necessary
  • services your insurance will cover from another home care agency, but not from Fairview Home Care and Hospice
  • services provided in a setting other than the patient’s residence
  • services provided to a patient who is not a Minnesota resident

Complete and mail your Fairview Home Care and Hospice Community Care application along with a status of your Medical Assistance Application to:
